We accept most major insurance.

Boston Hand to Shoulder will file claims with insurance companies for our patients.

We at Boston Hand to Shoulder try to stay informed about the ever-changing plans that are currently available, but it is impossible to remain current on all of them. Ultimately, it is your responsibility to keep informed of your own insurance coverage and any restrictions that may apply.

Insurance companies require that patients bring their insurance cards for all visits to verify identification.

It is always best to contact your insurance company with questions regarding coverage and participating providers. If you need to update your insurance information in our system, please call 866-489-4056.

Co-Payments

Co-payments are expected at the time of each visit and are collected when you check-in. For your convenience, payment can be made by cash, check, or credit card (MasterCard, Visa or American Express).

Referrals

Some insurance policies require that patients obtain a referral from their primary physician to Hand Surgery PC. If this is true for your insurance, please contact your Primary Care Physician to obtain the referral.

As your insurance coverage is a contract between you and your insurance provider, ultimately it is your responsibility to obtain a referral from your Primary Care Physician prior to your visit.

If there is no valid referral on file upon check-in, you will be asked to sign a waiver acknowledging that you are financially responsible for services rendered if insurance payment rejects as a result until your referral is received by our office.

There are insurance plans, like HMOs and Neighborhood Health Plan, which require a pre authorization referral to visit a medical specialist. After you schedule your appointment at Hand Surgery, PC, call your primary care physician to request that a referral be submitted to our office. Prior to your scheduled visit, we recommend that you call our office to verify that we have received the referral from your primary care physician. Patients with school insurance policies obtain health insurance referral forms from the school health office.

What if I have no insurance?

If a patient does not have insurance, an appointment can still be arranged.

Payment is expected at the time of your office. For your convenience we accept cash, check or major credit cards (MasterCard, Visa and American Express).

Workers Compensation Requirements

Hand Surgery, PC accepts most Workers’ Compensation insurances when you are being treated for an on-the-job injury. Please inform the appointment secretary that you have reported a work compensation injury to your employer when calling to make your appointment.

Prior authorization is required by your work compensation carrier before an appointment can be set up.

You must provide the secretary with the following information when you call so we can obtain prior authorization on your behalf:

  • Name of Workers' Compensation Insurance Carrier
  • Name of your Claim’s Adjuster, phone # and fax #
  • Claim Number
  • Date of Injury
  • Name, address and phone number of employer

It is the patient’s responsibility to have each visit approved by their adjuster per workers' compensation requirements before coming to your scheduled appointment.

Motor Vehicle / Personal Injury

We do see patients for motor vehicle related injuries. When calling for an appointment you must indicate that the injury is a result of a motor vehicle accident.

All motor vehicle related injury information must be reported to our billing department prior to scheduling an appointment. This will allow our billing department to secure confirmation from the motor vehicle insurance company. We also require your private health insurance information to assist with medical expense coverage as a result of the accident.
